01 The problem

Rehab after surgery happens at home, out of sight.

Once a patient is discharged, the exercise programme that decides how well they recover is done alone, between clinic visits. Doctors can't see whether sessions happen or whether they're done correctly, and patients lose momentum without anyone noticing until the next review.

One patient, at home
Seen by the care team: none of it. One patient's home programme: on day 3 a straight-leg raise done well, 10 of 10 reps; on day 9 the knee bends and the leg stops short, 6 of 10 reps; on day 16 the session is skipped. The next clinic review is on day 28. Non-adherence to physiotherapy exercise can be as high as 70%, and is often worst in unsupervised home programmes.

02 What we do

Smartan keeps the care team connected to every home session.

Smartan helps doctors monitor recovery remotely, and helps patients stay consistent with the rehab programme they were prescribed.

  • Completed
  • Flagged for review
  • Missed
  • Clinic review
Six weeks, with Smartan. Every home session is visible, including the missed ones, and the two that need a closer look are flagged.

For doctors: see every session, not just the review

Each home session arrives scored and summarised, so you know who is on track, who is slipping, and which movement needs attention before the next appointment.

For patients: a programme they can keep up

Patients follow each exercise with their therapist's recorded guidance and get clear feedback after every set, so staying on schedule feels manageable rather than lonely.

03 How it works

A phone camera, a vision model, and the clinician's own rules.

Patients record their exercises on an ordinary phone at home. Our vision models track how each joint moves, rebuild the movement in 3D, and check every frame against the rules their clinician set.

A dated list of every session, each with a trend line
  1. Record

The patient films the set on their phone, following the plan their clinician set. No sensors, wearables or markers.

  2. Track

More than 30 body keypoints are found in every frame of the video, so each joint's movement is followed through the whole set.

  3. Rebuild

The keypoints are fitted to a 3D body, so the session can be replayed on an avatar and viewed from any angle.

  4. Score

Every rep is scored against the prescribed movement and the patient's own baseline, and each fault is named with the reps it appeared in.

  5. Flag

Every session is logged, and only the ones that need the clinician's attention are flagged.

What the clinician sees

Each home session arrives rebuilt on the avatar and scored frame by frame, with the moments that need attention marked on the timeline.

Clinician dashboard
  • Video to avatar. The patient's phone clip, top left, rebuilt on a 3D body the clinician can turn.
  • A marked timeline. Red marks what must be corrected, orange what to keep an eye on.
  • A score for every frame. With the measurements behind it: angles, heights and holds.

04 In summary

What makes home rehab measurable

Eight parts, working together.

  • Markerless tracking

30+ body keypoints from ordinary phone video. No sensors or markers.

  • 3D body model

Each session replayed on an avatar, viewable from any angle.

  • Per-frame scoring

Every frame scored at 25 fps, joint by joint, across the whole set.

  • Joint-level risk

Three severity levels show which joint failed, and when.

  • Personal baseline

Progress measured against the patient's own clinic values.

  • Compensation detection

Asymmetry, guarding and fatigue flagged as they appear.

  • Exception-based review

Only patients whose movement changed are flagged.

  • Clinician voice guidance

Patients hear their own therapist's recorded cues.
